Complete an Online Membership Application

The CMG Membership Application can also be downloaded in PDF format. If you do not have the free Acrobat Reader software necessary to view and print it, you can download it here. If you are in the Atlanta area, please indicate the Greater Atlanta CMG region on the form.

Please complete the application, sign it and send it to CMG Headquarters. The completed application should be mailed along with all monies to CMG Headquarters at the address on the form. If payment is by credit card then the signed form can be faxed to CMGHQ at (856) 401-1708.

A newsletter written by members for members. Its purpose is to notify the membership of administrative issues and to provide information on regional activities, published three times a year.

CMG is in the process of providing some new online membership only services. Like our Resource Manual for finding and contacting other members. As well as access to our database of conference procedings and handouts.
